Dr. Tammy Blevins-Polson, DNP, APRN-C
I have been blessed with a career in nursing and healthcare that has spanned more than 40 years. My journey began while serving on active duty in the United States Air Force, where I was trained as an operating room scrub technician. That experience shaped my dedication to service and caring for others, values that have guided me throughout my life.
Along the way, I earned my Associate of Science in Nursing from Raymond Walters College, a Bachelor of Science in Nursing from the University of Cincinnati, a Master of Science in Nursing as a Nurse Practitioner from Indiana Wesleyan University, and a Doctor of Nursing Practice degree from Northern Kentucky University. Each step in my education has allowed me to grow both professionally and personally, deepening my passion for helping others live healthier, fuller lives.
My greatest joy has always been connecting with people — listening, supporting, and walking alongside them on their journey to better health. I believe in caring for the whole person and strive to create a space where every patient feels valued and understood. Outside of my profession, I am blessed with a wonderful family. I am married, the proud mother of four grown children, and “Mimi” to eight beautiful grandchildren who fill my heart with joy.

Stephanie Pruitt, BSN, RN
Stephanie is a registered nurse injector specializing in lip enhancement, neuromodulators, and hyaluronic acid dermal fillers. With over 10 years of ICU nursing experience, she brings advanced clinical skills and a strong commitment to patient safety into her aesthetics practice. Currently pursuing her degree as a Nurse Practitioner, Stephanie is passionate about helping patients look and feel rejuvenated through subtle, natural enhancements that leave them refreshed and confident.
Sally Arthur, RN
With over 12 years of specialized experience in aesthetic medicine, wellness, and weight loss, Sally Arthur, BSN, RN, AFPA Weight Loss Coach, brings a holistic approach to beauty and health. Combining advanced clinical expertise with a deep passion for helping others look and feel their best, she is committed to guiding clients on their journey toward radiant skin, balanced wellness, and sustainable confidence.
Known for her compassionate care and personalized approach, Sally believes that true beauty begins from within. She blends evidence-based treatments with lifestyle coaching to support her clients in achieving glowing skin, optimal health, and inner vitality.
Whether it's through aesthetic treatments, such as Botox and Fillers, laser, microneedling, tailored wellness programs, or weight management strategies, Sally is dedicated to empowering others to embrace their most vibrant selves—inside and out.
Stacie Carlson, Office Manager
Stacie is the cornerstone of Ageless Vitality Center, ensuring that every patient’s experience begins with warmth, professionalism, and care. As the first point of contact—whether by phone, email, or in person—she creates a welcoming and organized environment for all. Highly skilled and resourceful, Stacie manages the daily operations of the office, develops creative promotions, and ensures every detail is handled with precision. Known for her positive energy and approachable nature, she brings a sense of balance to the team, offering both efficiency and lightheartedness.
Felicia, Family Nurse Practitioner

Felicia is a board-certified Family Nurse Practitioner with extensive clinical experience across diverse healthcare settings. She offers a collaborative, patient-focused approach to care, supporting patients with hormone balance, preventive wellness, weight management, aesthetic services, and long-term vitality.
She combines evidence-based expertise with compassionate guidance, working closely with patients to tailor care plans that align with their lifestyle, goals, and priorities. Felicia partners with every patient to help them feel balanced, supported, and confident as they prioritize their long-term health and well-being.
Bert the Therapy Dog
Say hello to Bert, the heart and soul of Ageless Vitality Center! Bert is a little over a year old and proudly serves as our resident emotional support dog. Owned by our Office Administrator, Stacie Carlson, Bert is here to bring comfort, calm, and a few tail wags to your day..
